The following documentation must be submitted as a part of the admission packet for all students.
The completed Application for Admission. This should be completed and submitted as soon as possible, but no later than the day prior to the scheduled screening test date. Late or incomplete application paperwork will be kept on file in the event any additional screening test dates are scheduled.
$100 Admission Fee. This fee is due when the Application for Admission is submitted. This fee covers administrative testing costs and is non-refundable. In the case of acceptance, this fee does not apply toward the registration fee or tuition and fees.
A photocopy of the child's birth certificate.
Additionally for Pre-Kindergarten and Kindergarten
A photograph of the child. This photo will be kept by the school.
Additionally for 1st through 8th Grade
The completed Core Teacher Questionnaire. The teacher completing the form must have taught the child within the past 12 months. The evaluating teacher should mail this form directly to Holy Rosary School.
A photocopy of the child's latest report card.
Additionally for Catholic applicants
A photocopy of the child's baptism certificate.
A letter from the pastor verifying your active participation in your parish or a copy of the last calendar year's parish contribution statement. Your parish status affects your priority for admission.
